Knights News · IHSAA CONSENT & RELEASE REQUIRED


Due to the COVID-19 pandemic any student-athlete with a 2019-2020 physical on file with the SDHS athletic department will be able to compete in the 2020-2021 school year, with the same physical.  The IHSAA is requiring an additional Consent and Release Certificate.  That document can be accessed below and will be provided by any coach holding workouts in July.  This certificate must be signed by both the student and a parent.   For any athlete returning to workouts July 6, the certificate must be returned to the athletes coach before any participation can take place.  Student-athletes should bring the consent certificate with them the first day workouts resume.  Once school resumes the consent can be turned directly into the athletic department.  The date of the Last IHSAA Pre-Physical Examination for SDHS athletes that participated in the $20 physical day, was performed on April 2, 2019.

NOTE:  If any question on the questionnaire requires an answer of (Yes), then the student-athlete must have a new 2020-2021 Pre-Participation Physical Examination form completed before participation can resume.
